Authorization Review Criteria

Effective January 1, 2024, Medicare requires that any criteria used to approve or deny authorizations must be available to the public. To access the criteria, please follow these steps:

1. Click on the link, https://prod.ds.interqual.com/service/connect/transparency?tid=2c54a087-7858-4078-a875-c4ebe3bd90fd
2. Click on Create One Healthcare ID and fill out the required fields.
3. Click continue and you will be provided a verification code via email.
4. Enter verification code when prompted and click verify.
5. Verify phone number by clicking via text or call, or skip the process.
6. If you choose to verify your phone number, enter the verification code when prompted.
7. Acknowledge the privacy agreement.
8. Access criteria by clicking on ALL PRODUCTS.
